Certified copies of Birth and Death certificates can only be issued to family members, legal guardians and certain court-related parties and legal representatives. Applicants must complete an application and show acceptable identification.

To obtain a copy of any Vermont birth, death or marriage certificate, you may:
Come into the office, complete the application, provide valid ID, and we will get your certificate immediately, or
Mail a completed application, $10.00 check or money order and a copy of your valid ID to:
